New Street or Facility Name Nomination

The following is a Nomination Form for a new street or facility name for consideration for addition to the Master Name List, and may be selected at a future date for use as a street or facility.

All submissions should be compliant with the Street and Facility Naming Policy.

This form will be considered by the Mayor, a local City Councillor(s), and City Staff after submission. You will be notified if you nomination is accepted and if you nominated name is selected for use.

Personal information contained on this form is collected pursuant to The Municipal Act, 2001, and will be used for the purpose of providing feedback to City Council.  Questions about the collection of this information should be directed to the City Clerk or Deputy Clerk at clerks@kawarthalakes.ca.
