Municipal Address Sign Request Form

This form is to be used when ordering a replacement sign or a new sign not associated with a building permit.

Mandatory:

A site sketch is required to accompany your application, clearly showing the location of the entrance and the nearest cross roads and existing neighbouring numbers. 

Note: If the entrance is not constructed, the location must be clearly marked in the field by white stakes.

The information provided on this form is true to the best of my knowledge. I have authority to act on behalf of the owner, corporation or partnership with respect to this application (if applicable).

Please Note: Upon completion of this form a signed copy must be submitted to the Building & Septic Division in conjunction with a site sketch and the appropriate fee.

Print, sign and submit to the Building & Septic Division, via email, drop off at any municipal Service Centre, mail to the address at the top of this form or alternatively email to buildingadmin@kawarthalakes.ca
